Customize Subscription Form
NOTE: You cannot make changes to a web form once you create and save the web form.
Enter a name that identifies the web form in the Name text box.
Select the group that this web form should populate from the Group drop-down list.
Select the fields that you wish the subscriber see on the form by checking the Display checkbox for that field. The following fields are available for use on web forms:
Occupation
Company
Address Line 1
Address Line 2
City
State/Province
Postal Code
Country
Phone 1
Phone 2
Fax
Website
Birthday
Gender
Preferred Mail Format
Additional Notes
User Defined Field 1
User Defined Field 2
User Defined Field 3
User Defined Field 4
User Defined Field 5
User Defined Field 6
User Defined Field 7
User Defined Field 8
User Defined Field 9
Select the fields that the subscriber MUST fill out for the form to be accepted by checking the Required checkbox for that field.
NOTE: Requiring fields may discourage people from filling out your form and subscribing. Make sure that the fields you are requiring are absolutely essential to your business. By default, only the email address is required. Because email is automatically included on every web form, it is not on this list.
Specify the font color for the form by clicking the color picker button and selecting the color you want for the text.
Specify the background color of the form by clicking the color picker button and selecting the color you want for the background.
Include an image to go at the top of the form (optional):
Click the Insert Image button.
You will then be prompted to select one of your existing graphics, or upload a new graphic, in the Browse For Image screen.
Click the Save and Preview button to save the form and view your changes.
Click the OK button, and the form will be saved to the list of web forms on the Web Forms screen. If you are not satisfied with the web form you have just created, click the Cancel button to delete the form.
